Practice closing
We would like to inform you that MountainStar Providence Family Medicine will be closing, effective June 1, 2024. The Ogden Clinic Professional Corporation (“Ogden Clinic”) is purchasing assets and will operate under a new clinic name at the existing location.
MountainStar Providence Family Medicine is appreciative of the trust you have shown us in allowing our physicians to provide for your medical needs. We would like to inform you that MountainStar Providence Family Medicine will be closing, effective June 1, 2024. The Ogden Clinic Professional Corporation (“Ogden Clinic”) is purchasing assets and will operate under a new clinic name at the existing location 382 W 280 N, Providence, UT 84332, effective June 2nd, 2024.
Dr. Ryan Allen and Dr. David Johnson will no longer be practicing at MountainStar Providence Family Medicine, effective June 2, 2024.
Dr. Richard T. Stevens and Dr. James Mathews will begin working for Ogden Clinic as of June 2, 2024, and Ogden Clinic will become the custodian of their patients’ confidential medical records as of June 12, 2024.
We would like to make this transition as easy as possible for you. As such, there are a few options for you to consider for your future healthcare needs.
If you would like to receive a copy of your records to transfer to a new provider, please complete the included patient authorization form, sign and return to MountainStar Providence Family Medicine, and we would be happy to assist you. As of June 12, 2024, Ogden Clinic will be the custodian of your confidential medical records and will continue to operate using this telephone number.
